Before any treatment, you will be asked to review and sign a separate Informed Consent for Chiropractic Care. That document — not this page — governs the specifics of your care. By scheduling, you acknowledge that chiropractic care, like any healthcare intervention, carries risks, and you agree to discuss those risks with Dr. Dakota during your consultation.
4 · Appointments & Cancellation
- Appointments may be booked online, by phone, or in person.
- We request 24 hours' notice for cancellations or rescheduling so we can offer the slot to patients on our waitlist.
- Late cancellations or no-shows may incur a $50 fee, charged to the card on file.
- First-visit no-shows forfeit any prepaid consultation fee.
- We reserve the right to decline future appointments after repeated no-shows.
5 · Payment & Insurance
Payment is due at the time of service unless other arrangements have been made. We accept cash, check, all major credit cards, HSA/FSA, and most major medical insurance. For care plans extending beyond initial active care, in-house payment plans and CareCredit financing are available for qualifying patients. You are ultimately responsible for charges your insurance does not cover.
6 · Liability
To the maximum extent permitted by law, Envision Chiropractic disclaims liability for indirect, incidental, or consequential damages arising from your use of this website. Nothing in these terms limits liability that cannot be limited under applicable Washington State law, including liability for personal injury caused by negligence.
